A signed referral (script) from your physician is required for therapy services. If you already have a signed prescription, please email it to bt@beyondtherapy.care or fax it directly to 979-743-2109
This includes medical history, consent forms, and sensory profiles (if requested by your therapist). Medical history and consent forms are available through our patient portal. To set up your portal access, please contact our office with your email address.
